Unfortunately we have no method of objectifying tinnitus. For some persons, they wake up in the middle of the night with a buzzing or ringing, feeling dizzy, and not hearing well. This is a Meniere's attack. For others, they wake up one day with tinnitus. They wait a day or so and it doesn't go away. Now they are worried, and anxious and it feels like it is getting worse. The more they worry, the worse the symptoms. This is because of the fight/flight response. We are all born to the fight/flight response to dangerous sounds. For most persons, the tinnitus is identified with the sound of danger, and this starts the anxiety cycle. The commonest cause of tinnitus is industrial noise trauma that causes hearing loss and then the person hears the "FM carrier wave" that the body generates for hearing. Why does one person get tinnitus, yet the other person with the identical trauma and hearing loss not get it? Current Theories One theory for tinnitus is that certain hair cells in the inner ear are no longer working, so the brain makes up for that absent sound, generates the missing sound. Another theory is that in the brain there is a braking mechanism that keeps loud sounds from hurting you. Something damages that brake set up and now ordinary sounds and impulses are magnified. This theory accounts for why hyperacusis may be associated with tinnitus. Another theory is that excess impulses arise in the course of hearing and that extra impulses can affect these signals and you get the tinnitus sensation. Thus, muscle input from the neck after a whiplash does this. You have a sound generator, that created a current. Like an FM carrier radio wave. When sound affects that FM carrier wave you hear music. In Tinnitus you are hearing that carrier wave. None of the theories explain well why normal hearing persons get tinnitus, or why one person gets it and under identical circumstances their identical twin doesn't. Or why a person with no hearing problem gets his auditory nerve severed, is now deaf, and now has tinnitus. What is new is that we find that persons with severe hearing loss and tinnitus who get a cochlear implant with improved hearing, now may be free of tinnitus.
Slide 2: Tinnitus affects ten million persons because of multiple causes: Hypertension Middle and inner ear infections Meniere's disease Acoustic Trauma including rock concerts and loud MP3 players Industrial noises-construction and manufacturing Migraine Temporomandibular Joint Disease Somatosensory -from neck muscles, whiplash Increased fluid pressure in brain Cochlear Hydrops Ototoxic drugs Pressure on the nerves of hearing Medications such as aspirin salycilates. What works for tinnitus? Despite absence of objective tests for tinnitus, patients can be helped. Persons who suddenly develop tinnitus are subject to severe anxiety reinforcement and are relieved by taking away the anxiety. The Tiger Outside the Cave. Persons are born with a flight/stress reflex. Our ancestors heard the twig snap outside the cave. That was a signal that the tiger was out there and you must flee. Those that stopped to inspect and analyze the sound didn't stay around to procreate. When you mash your finger, it hurts and you cry out "OW." If you are lucky, mother will be there to kiss the finger and make it better. Then there is no anxiety to reinforce the pain. Even with no one to comfort you, the mashed finger or broken bone doesn't automatically trigger a stress reaction. But tinnitus does. When you hear this strange high tone ringing sound in your ear that you know is not produced by the TV, you get the same fight/ flee response that saved our ancestors. Without a clear cognition, you continue to get the stress-anxiety reinforcement cycle. The more the anxiety, the more nervous you get, the more the tinnitus bothers you and the more nervous you get. Jim has tinnitus but it is masked. Jim Smith lives in Manhattan on the second floor. He is accustomed to the cars, the sirens, and the street noises at night, and he sleeps soundly. Now, he is driving to Chicago and stops at a small motel way out in the country where it is QUIET. In the quiet, he now hears a ringing in his ears/head, which was masked before. He gets the tiger reaction. His body is receiving adrenalin. Glycogen-the brain food-is shunted to his muscles. He can't sleep so he heads back home. He goes to the emergency room and they tell him to see his doctor. Takes a week to see his doctor, who then says he needs to see a specialist. While waiting, he is stressed and anxious. The more
Slide 3: he worries, the more nervous he gets, the louder the tinnitus seems to be. Poor Jim must wait another week to see the specialist who then refers him for audiology and an MRI. These also must be scheduled days later. Finally, he sees the specialist who tells him that he doesn't have a tumor and therefore he doesn't need surgery and not to worry. Unfortunately the doctor doesn't give Jim any sort of treatment plan, at least to reduce his anxiety circle. Jim is now transformed into a stressed individual who suffers metabolic disease from too much stress. With excess adrenaline and other products, his heart and blood vessels may be damaged. As a result, Jim is not doing well. His body says tiger and continues to feel stressed. His wife says he should relax; but how to relax when you body says there is a tiger prowling outside your cave? The Importance of Taking Charge Since tinnitus automatically triggers the tiger sound/stress reaction, therapy is directed at reducing that stress reaction. Many persons are told by their doctor to forget about the tinnitus sound, or just learn to ignore the sound. Patients may do poorly with this advice. Imagine you are in the cave and outside you hear the tiger and you are told to just ignore it! Therapy that works. Stop the stress Because the tinnitus is sending constant stress/tiger signals to your stress center, what is needed is to change the signals. Send NO TIGER signals instead. When you are stressed, you breathe rapidly and this tells the stress center-yes tiger. Therefore if you change you breathing to one of slow exhalation, you signal no tiger. To yourself, quietly, breathe in count of four and out count of six. By making your exhale a relaxed one, the stress center gets a signal that it recognizes as no tiger, no stressor. When you are stressed, your muscles tighten and glycogen shifts to the tight muscles so that you can run or fight. Therefore, if you look in the mirror and SEE your face, your jaw and your shoulders relax, now the stress center is signaled -no tiger. For most patients with tinnitus or hyperacusis, using the mirror to relax your muscles correctly, ten minutes a day, is successful therapy. With relaxation the tinnitus becomes like a tight bra or belt you learn to ignore. Remember when you were fully relaxed and feeling good? That day at the beach, after a massage, a party you enjoyed, or a time when you received that prize. At those good moments you felt good and relaxed and your physiology was at its peak. You can replicate that good feeling and good physiology by recalling that good experience using ALL your senses plus joy. If you see, hear, smell, taste, and feel of that moment, you will get all the same physiology. With these three signals-the relaxed breathing, the relaxed muscles-the mirror shows you when you do it right, and with visualization using all senses, the stress center stops the outflow of adrenalin and other elements. With this relaxed state, the tinnitus is not reinforced and becomes like a simple itch or tight shoe you can ignore. Practice makes perfect
Slide 4: These actions must be practiced regularly to achieve best results. In the Tinnitus chapter in the book, Stressed? Anxiety? Your Cure is in the Mirror are the recommended exercise that makes stress reduction as automatic as pressing on the brake with your right foot when you see a red light. Martin has tinnitus but has no symptoms How is Jim's sequence different from Martin, a three year Police officer? Martin has been sloppy about using ear protection on the firing range. He is aware that lately he has this sound in his right ear. He isn't sure whether to go to the company clinic or a private doctor. He asks his mentor, "Old Sarge" about this. Old Sarge assures him, "Hey man, forget about it. Everyone one of us gets it. It comes with the territory. Now Martin is reassured that this is not a tiger and so he avoids the stress reaction; more important, he doesn't get the anxiety reinforcement. By Cognition, he has learned that the tinnitus is not a "bad." When he thinks about it, it is there, but without the stressors, it is like a scar on his leg or the annoying weight of his gun belt.
